Caring Professionals provides home care across all five boroughs of New York City—the Bronx, Brooklyn, Manhattan, Queens, and Staten Island—as well as Nassau and Westchester counties. We have offices in the Bronx, Brooklyn, Queens, Westchester, and Nassau County.
A Licensed Home Care Services Agency, or LHCSA, is a home care agency licensed by New York State to provide personal care services in the home. This includes assistance with daily living activities such as bathing, dressing, meal preparation, and medication reminders. The LHCSA employs the Home Health Aides and Personal Care Assistants who deliver this care. Caring Professionals is a New York State licensed LHCSA.
A Certified Home Health Agency (CHHA) provides skilled, short-term services following a medical episode such as a hospitalization. These services include occupational therapy, physical therapy, speech therapy, wound care, and social work. The CHHA coordinates overall care and works with a Licensed Home Care Services Agency (LHCSA) like Caring Professionals to provide the Home Health Aide who assists the patient at home. In short, the CHHA manages the clinical services; the LHCSA provides the daily personal care.
When a patient is discharged from a hospital or rehabilitation facility, the hospital’s discharge planner or social worker arranges short-term care through a Certified Home Health Agency (CHHA). The CHHA then coordinates with a Licensed Home Care Services Agency to provide a Home Health Aide. A CHHA nurse typically visits the patient’s home the day after discharge to assess needs and set up services. You can ask for Caring Professionals by name during the discharge process.
Medicaid eligibility in New York is determined by New York State based on financial and medical criteria. For long-term home care, Medicaid recipients enroll with a Managed Long Term Care (MLTC) plan, which manages their health services including home care. A Maximus nurse assessment is part of this process. Caring Professionals can assist families with Medicaid enrollment and guide you through the MLTC selection process.
You can still receive home care from Caring Professionals. We work with families who have Medicare, long-term care insurance, or who are paying privately. Our intake team can discuss the options that fit your situation and, if appropriate, assist with a Medicaid application.
Our Home Health Aides assist with activities of daily living including personal care, bathing, dressing, meal preparation, medication reminders, light housekeeping, accompaniment to appointments, shopping, and participation in community activities. Care can be provided in your home, an assisted living facility, or a rehabilitation center.
