The cost depends on the type of care, the number of hours, and how the care is funded. Many Long Island families receive home care through Medicaid and a Managed Long Term Care (MLTC) plan at no out-of-pocket cost. We also offer private pay options and accept long-term care insurance. Long Island residents apply through their county, not New York City’s HRA. In Nassau, contact the Nassau County Department of Social Services at 60 Charles Lindbergh Boulevard, Uniondale, (516) 227-8519. In Suffolk, applicants 65 and over or with a disability apply through the Department of Social Services Medicaid centers in Hauppauge, (631) 853-8408, or Riverhead, (631) 852-3570. Once Medicaid is in place, the New York Independent Assessor (NYIA) determines eligibility for long-term home care, and you choose an MLTC plan. Medicare covers short-term skilled services after a hospitalization or medical episode, including home health aide visits, physical therapy, occupational therapy, and speech therapy. These are coordinated through a Certified Home Health Agency (CHHA). For longer-term home care, Medicaid through an MLTC plan is the most common pathway. Private pay home care starts quickly and can be scheduled for as few or as many hours as your family needs, and long-term care insurance often covers it. Residents 60 and over may also be eligible for EISEP in-home services through the Nassau County Office for the Aging, (516) 227-8900, or the Suffolk County Office for the Aging, (631) 853-8200.
In urgent situations, such as a discharge from NYU Langone Hospital—Long Island, Nassau University Medical Center, or Stony Brook University Hospital, we can often begin care within 24 to 48 hours. For planned care, we typically complete the needs assessment and caregiver matching within a few days. The timeline also depends on insurance authorization if you are using Medicaid or an MLTC plan.
