## Experienced Home Health Aides

For over 30 years, our mission has been to offer qualified home care support that is also knowledgeable of customs, languages and personal needs. We train our people to establish a deep connection. We give them the tools to encourage a close relationship.

Our dedicated caregivers have decades of home care and nurturing experience behind them.
They integrate into your home, your culture and your own environment with respect.

## Managed Long Term Care

Medicaid recipients can qualify for long term home care through New York State’s system of care for the elderly and people with disabilities.
Qualifying for this system involves a visit/telehealth visit by the Maximus nurse. The family then chooses a managed long term care company which will be responsible for all aspects of maintaining the health of the patient. The home care will be delivered by the licensed home care services agency, Caring Professionals.

Caring Professionals can assist families through this entire process, and provide ongoing support to explain necessary paperwork, assessments and enrollment procedures:

							-

How to request the Maximus nurse visit

## Short Term Care

Medicare recipients may qualify for short term home care as well as skilled services after a medical episode. These are arranged by the discharge planner (social worker) in the hospital or rehab center.

The CHHA (Certified Home Health Agency) has overall responsibility for all the services in the home, including occupational therapy, physical therapy, speech therapy, wound care and social work.

The home care agency provides the home health aide (HHA) in coordination with the CHHA.
Caring Professionals can assist families in navigating the discharge to the home, and ensuring that all necessary services are seamlessly provided.

					Coordination with the CHHA

If you do not have Medicaid, you may still qualify for this free wrap around coverage. Our intake specialists can direct you to a quality Medicaid enrollment service which will not charge for its service.

Please note: Caring Professionals does not make decisions on application approvals nor does it determine hours of home care. We just deliver quality home care.

If you are determined as ineligible for Medicaid, then please get in touch to discuss how we can provide you with private pay quality home care.

New York State’s eligibility rules are still inclusive, and are designed to help seniors and people with disabilities avoid exhausting all their resources on health care.

When my mother-in-law was not getting enough hours to deal with her nighttime disorder, Caring Professionals advised us how to apply for an assessment by the appropriate MLTC. She now has adequate coverage and wonderful aides. We can sleep at night knowing that she won’t injure herself like she has in the past.

Taking care of my grandfather has long been the role of my uncle. Caring Professionals not only informed me that my uncle could get paid for what he does, but they patiently coached him through the enrollment process. Now the whole family is so much happier about my grandfather’s aging.

What is the difference between an HHA and a PCA?

A home health aide completes more training than a personal care aide. In New York, PCA certification requires a minimum of 40 hours; HHA certification requires a minimum of 75 hours, including 16 hours of supervised practical training. A certified PCA can upgrade to HHA by completing an additional 35 hours rather than starting over.

The practical difference is scope. Both help with daily activities like bathing, dressing, meals and mobility. An HHA can also carry out certain health-related tasks under a nurse’s supervision, such as helping with prescribed exercises or assisting with medication, which a PCA is not certified to do.

How do I apply to work as a Home Health Aide or Personal Care Assistant at Caring Professionals?

When applying in person you will need (as per New York State regulations) an active HHA or PCA certificate, a pre-employment physical and labs, a legal form of ID and social security card (no photocopies), and a direct deposit form or voided check.
